29 CFR 1926.102(a)(1): The employer did not ensure that each affected employee used appropriate eye or face protection when exposed to eye or face hazards from flying particles.  The employer is failing to protect employees from eye injuries when operating a pneumatic nail gun.  This was most recently observed at 2503 Century Rd, Bellevue, Nebraska on July 31, 2018 when an employee was observed using a pneumatic nail gun and they were not using eye protection.

29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system:  The employer is failing to protect employees from fall hazards while performing residential roofing activities.  This was most recently observed at 2503 Century Rd, Bellevue, Nebraska when 4 employees were observed working on the roof of a residential structure and they were not using fall protection.

29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed:  The employer is failing to protect employees from fall hazards while utilizing ladders to access a residential roof.  This was most recently observed at 2503 Century Rd, Bellevue, Nebraska on July 31, 2018 when employees were observed using a ladder to access the roof of a residential structure and the ladder did not extend at least 3' past the roof edge.
