29 CFR 1926.404(f)(6): The path to ground from circuits, equipment, or enclosures was not permanent and continuous: a) On or about 7/27/18, on the second floor inside the Five Story Concrete section of a building under renovation at 79 Perry Street, Buffalo, NY: An Oneida Air Systems, Dust Cobra Vacuum Cleaner was connected to 120 volt-ac power via an extension cord. The plug on the extension cord lacked a ground prong. b) On or about 7/27/18, on the second floor inside the Five Story Concrete section of a building under renovation at 79 Perry Street, Buffalo, NY: A second extension cord was plugged into a 120 volt-ac receptacle. The plug on that extension cord also lacked a ground prong, it was laying on the floor and was available to power any equipment anytime. NO ABATEMENT CERTIFICATION REQUIRED

29 CFR 1926.416(e)(1): Worn or frayed electric cords or cables shall not be used: a) On or about 7/27/18, on the second floor inside the Five Story Concrete section of a building under renovation at 79 Perry Street , Buffalo, NY: The cord on the Oneida Air Systems, Dust Cobra Vacuum Cleaner was frayed at the point where it entered the plug, exposing the conductors. ABATEMENT CERTIFICATION REQUIRED
