29 CFR 1910.253(b)(4)(iii): Oxygen cylinders in storage were not separated from fuel-gas cylinders or combustible materials (especially oil or grease), a minimum distance of 20 feet (6.1 m) or by a noncombustible barrier at least 5 feet (1.5 m) high having a fire-resistance rating of at least. (a) 379 Alicia Road, East Millsboro, PA 15433, On or about August 2, 2018, near the facility outside storage yard, oxygen and fuel gas compressed gas cylinders were being stored together. The cylinders were approximately 5 feet apart and were not separated by a fire rated barrier.

29 CFR 1915.74(c)(2): - A ramp meeting the requirements of 29 CFR 1915.74(c)(1) was not provided where employees had to step from a wharf, float, barge, or river towboat, and employees could not step safely on to the vessel. a) Vessel: Colin Robert Murray, pier side, 379 Alicia Road, East Millsboro, PA 15433, on or about August 2, 2018, employees were exposed to fall hazards due to the use of an A-Frame ladder as means of access from the dock to the vessel. The ladder also did not extend at least 36 inches above the upper landing surface on the dock.
