On or about 10/08/18, the employer failed to submit documentation and certification of corrective action for citation 1 item 1 and citation 1, item 2 issued on 9/21/18 with an abatement due date of 10/18/18.

29 CFR 1926.501(b)(1): Each employee on a walking/working surface with an unprotected side or edge which was 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems:  On or about August 1, 2018, at the north side of the jobsite, employees engaged in commercial construction operations were exposed to a 9-foot fall hazard without means of fall protection.

29 CFR 1926.503(a)(1): The employer did not provide a training program for each employee potentially exposed to fall hazards to enable each employee to recognize the hazards of falling and the procedures to be followed in order to minimize these hazards:  On or about August 1, 2018, at the north side of the jobsite, employees who had not received fall protection training while engaged in commercial construction operations were exposed to a 9-foot fall hazard without means of fall protection.

29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed:  On or about August 1, 2018, at the south side of the jobsite, employees were exposed to a 9-foot fall hazard when the ladder used to gain access to the work level was not extended 3 feet over the landing surface.
