29 CFR 1910.212(a)(3)(ii): Point(s) of operation of machinery were not guarded to prevent employee(s) from having any part of their body in the danger zone(s) during operating cycle(s):  Facility: The ingoing point of operation (nip point) created between the Feed Roll and the Center Roll on the Day 14" X 30" Three Roll Mill was not safeguarded in that the high modulus polyethylene (HMPE) allowed up to 4 1/2 inches of unprotected area on the right side of the rollers and up to 4 3/8 inches of unprotected area on the left side of the rollers along with an inefficient safety pressure bar that upon initiation allowed the rollers to continue movement for up to an additional 30 seconds.  Consequently, a worker, while cleaning the running rollers, sustained serious hand injuries after their hand was pulled into the ingoing point of operation (nip point) rollers.
