The program shall enable each employee to recognize the hazards of falling and shall train each employee in the procedures to be followed in order to minimize these hazards.  The employer failed to ensure that employees are adequately trained by a competent person in the standards of this subpart resulting in employees failing to utilize proper fall protection. This was most recently observed at the jobsite located at or about 1406 Greenwood Ave Papillion, NE 68133. The employer failed to ensure that ladders extended 3 feet above the landing surface when using portable ladders in the performance of roofing work.
