On or about August 3, 2018, employees removing siding from a residence located at 920 Jackson St Oshkosh, WI did not keep the work area clear of debris and scrap lumber with protruding nails. The area underneath and around the ladder jack scaffold next to the residence's north entrance was covered with scrap and siding that had protruding nails.

Non-mandatory Appendix A to this subpart contains examples of criteria that will enable an employer to comply with paragraph (a) of this section. a. On or about August 3, 2018, employees removing siding from a residence located at 920 Jackson St Oshkosh, WI were on a ladder jack scaffold that was not designed in accordance with criteria contained in paragraph (a) of this section. The working surface was not scaffold grade planking or engineered material, the West side of the scaffold was not supported by scaffold components and was not secured to the soffit section of the home that it was bearing on.
