29 CFR 1926.501(b)(1): Each employee on a walking/working surface with an unprotected side or edge which was 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems.    On or about July 17, 2018, at Lennar at Park Central, 10651 NW 88th Street, building 7, Apex, Doral, FL 33178, employees who were wearing a PFAS harness, but not connected to an anchor point while working on a roof-top applying stucco/plaster(a walking/working surface) were not protected from falling by the use of guardrail systems, safety net systems or personal fall arrest systems; thus exposing the employees to an approximate 36-foot fall hazards.    NOTE: CERTIFICATION AND DOCUMENTATION OF ABATEMENT IS REQUIRED FOR THIS VIOLATION.

29 CFR 1926.502(d)(21): Personal fall arrest systems were not inspected prior to each use for wear, damage, and other deterioration, and/or defective components were not removed from service:    On or about July 17, 2018, at Lennar at Park Central, 10651 NW 88th Street, building 7, Apex, Doral, FL 33178, the employer failed to ensure that Personal Fall Arrest Systems which had been impact-loaded, and also had signs of heat and sun/weather related damage and other deterioration were inspected and removed from service; thus exposing the employees to a fall hazard.      NOTE: CERTIFICATION AND DOCUMENTATION OF ABATEMENT IS REQUIRED FOR THIS VIOLATION.

On or about July 17, 2018, at Lennar at Park Central, 10651 NW 88th Street, building 7, Apex, Doral, FL 33178, employees working/apply stucco from a supported scaffold, which was positioned on a rooftop more than ten feet above the lower level/ground and was not fully planked or decked between the front uprights and guardrail supports; thus exposing the employees to an approximate 36-foot fall hazard.    b. On or about July 17, 2018, at Lennar at Park Central, 10651 NW 88th Street, building 7, Apex, Doral, FL 33178, employees working/apply stucco from a supported scaffold, which was positioned on a breezeway more than ten feet above the lower level/ground and was not fully planked or decked between the front uprights and guardrail supports; thus exposing the employees to an approximate 24-foot fall hazard.     P & W Plastering Corp. was previously cited for a violation of this Occupational Safety and Health Standard 29 CFR. 1926.451(b)(1) or its equivalent standard, which was contained in OSHA inspection number 1257357, citation number 01, Item number 001a, and was affirmed as a final order on 03/27/2018, with respect to a workplace located at 15400 NW 97th Avenue, Hialeah, FL 33018.     NOTE: CERTIFICATION AND DOCUMENTATION OF ABATEMENT IS REQUIRED FOR THIS VIOLATION.
