29 CFR 1926.501(b)(1): Each employee on a walking/working surface with an unprotected side or edge which was 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems. (a) At jobsite, on or about and at times preceding to July 31, 2018 employees were exposed to fall hazards of up to 12.4 feet during painting activities on the club house roof trim.

29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed. (a) At jobsite, Club House Roof, on or about, and at times preceding to July 31, 2018, the extension ladder used for accessing the roof area extended 1.5 feet beyond the roof line, creating a 12.4 foot fall hazard.
