Broad Street, East Bangor, Worksite - On or about August 6, 2018, employees were performing residential roofing work while working from a ladder jack scaffold, set on a lower roof level approximately 18 feet above grade, where one of the scaffold platforms was bridged to a second platform above a single supporting ladder jack.
