On or about July 21, 2018 three employees were installing pipe for a waterline in an 8 feet to 10 feet deep excavation containing between 1 foot to 2 feet of water at the bottom.

On or about July 21, 2018 three employees were working in an excavation that extended below the bottom depth of a concrete utility vault structure. One or more methods were not used to ensure the stability of the vault structure.

On or about July 21, 2018 the competent person who inspected the excavation permitted three employees to work in an 8 feet to 10 feet deep excavation without the use of adequate protective systems, and with water accumulated in the bottom of the excavation.
