On or about July 17, 2018, at Lennar at Park Central, 10651 NW 88th Street, building 8, Summit, Doral, FL 33178, the employer failed to ensure that each employee on a supported scaffold more than 10 feet (3.1 m) above a lower level were protected from falling to that lower level by the use of a personal fall arrest system or a guardrail system on all three sides when employees were erecting a concrete block wall from the scaffolds; thus exposing the employees to a 10"2" fall hazard.     NOTE: CERTIFICATION AND DOCUMENTATION OF ABATEMENT IS REQUIRED FOR THIS VIOLATION.
