29 CFR 1910.1200(h)(1):   Employees were not provided effective information and training as specified in 29 CFR 1910.1200(h)(2) and (h)(3) on hazardous chemicals in their work area at the time of their initial assignment and whenever a new hazard that the employees had not been previously trained about was introduced into their work area:  As of August 2, 2018, at least two employees who were exposed to hazardous chemicals (e.g., solvents, spray paint, glass cleaner, refrigerant, motor oil, coolant) while performing duties at the Columbia Metropolitan Airport had not been trained on the new chemical container label elements and the new format and types of information on each section of safety data sheets (SDSs) with respect to revisions to the standard in accordance with the Globally Harmonized System of Classification and Labeling of Chemicals (GHS).  ABATEMENT CERTIFICATION IS REQUIRED

29 CFR 1910.146(c)(2): The employer did not inform exposed employees, by posting danger signs or by any other equally effective means, of the existence and location of and the danger posed by the permit spaces:  (a) The access to an underground water valve vault located adjacent to an entrance driveway adjacent to the air traffic control tower was not provided with a danger sign to identify the location and danger posed by the space.   (b) Danger signs were not posted at two underground communication vaults located adjacent to the Columbia District System Support Center office at the air traffic control tower.      NOTE: A sign reading DANGER-PERMIT-REQUIRED CONFINED SPACE, DO NOT ENTER or using other similar language would satisfy the requirement for a sign.
