29 CFR 1910.134(g)(1)(i)(A) as referenced in 29 CFR 1926.103: : Respirators with tight-fitting facepieces were worn by employees who had facial hair that came between the sealing surface of the facepiece and the face or that interfered with valve function: On or about August 2, 2018 and times prior thereto, employees doing masonry work, to include but not limited to cutting cinder blocks, are required to wear tight fitting facepeices had facial hair that would interfere with the seal. This condition exposed employees to inhalation hazards.

29 CFR 1926.501(b)(6): Each employee on ramps, runways, and other walkways was not protected from falling 6 feet (1.8 m) or more to lower levels by guardrail systems: On or about August 2, 2018 and times prior thereto, employees were exposed to falls greater than 6 feet due to unprotected sides along the dirt pathway next to the retaining wall where the training center will be built. This condition exposes employees to fall from heights hazards.
