29 CFR 1926.501(b)(15): "Walking/working surfaces not otherwise addressed." Except as provided in 1926.500(a)(2) or in 1926.501 (b)(1) through (b)(14), each employee on a walking/working surface 6 feet (1.8 m) or more above lower levels shall be protected from falling by a guardrail system, safety net system, or personal fall arrest system. On or about August 6, 2018, during the installation of concrete shoring and carpenter bracket scaffolding for the construction of an elementary school building, located at 27555 Center Ridge Road, Westlake, Ohio, the employer did not ensure three employees working from concrete shores (walking/working surface) were protected from falling approximately 14.5 feet, by utilizing a guardrail system, safety net system, and/or personal fall arrest system.
