29 CFR 1910.140(c)(10)(i): Snaphooks and carabiners were connected directly to webbing, rope, or wire rope;     On or about August 3, 2018, employees were exposed to fall hazards up to and in excess of 40 feet without fall protection, while erecting a ride/coaster, at the Illinois State Fair in Springfield, Illinois.   In an attempt to provide fall protection, employees wrapped their lanyard around the structure and snapped it backed to itself/lanyard.

29 CFR 1926.454(a): The employer did not have each employee who performed work while on a scaffold trained by a person qualified in the subject matter to recognize the hazards associated with the type of scaffold being used and to understand the procedures to control or minimize those hazards:  On or about August 3, 2018, employees were exposed to fall hazards up to and in excess of 40 feet without fall protection, while accessing the working level of the elevated work platform, at the Illinois State Fair in Springfield, Illinois.   The employer did not provide scaffold training.  This training would include:    a.         Nature of any electrical hazards, fall hazards, and falling object hazards in the work area, b.         Correct procedures for dealing with electrical hazards and for erecting, maintaining, and disassembling the fall protection systems and falling object protection systems used, c.         Proper use of the scaffold, and the proper handling of material on the scaffold,  d.         The maximum intended load and the load-carrying capacities of the scaffold used, e.         Any other pertinent requirements of this subpart, L  Scaffolds.

29 CFR 1926.451(e)(1): When scaffold platforms were more than 2 feet (0.6 m) above or below a point of access, portable ladders, hook-on ladders, attachable ladders, stair towers (scaffold stairways/towers), stairway-type ladders (such as ladder stands), ramps, walkways, integral prefabricated scaffold access, or direct access from other scaffold, structure, personnel hoist, or similar surface was not used.  Crossbraces were used as a means of access:    On or about August 3, 2018, an employee was exposed to fall hazards up to and in excess of 40 feet without fall protection, while accessing the working level of the elevated work platform, at the Illinois State Fair in Springfield, Illinois.   The employee climbed the vertical structure on which the coaster track would be connected to reach the elevated work level/platform.

The employees walked up the coaster sections without fall protection, in order to reach the work level/platform.
