29 CFR 1926.501(b)(5): Each employee on the face of formwork or reinforcing steel was not protected from falling 6 feet (1.8 m) or more to lower levels by personal fall arrest systems, safety net systems, or positioning device systems: a) 15th floor, rebar holding rack: Employees climbed to the top of the rebar piles to tie rebar into bundles and sort the bundles on the rack. An employee climbed at the south end of the rack, which was close to the edge of the building. The employee was at a walking/working level which the guardrail system could not provide adequate fall protection. The employee was exposed to falling 15 stories from the edge of the building. On or about 08/07/18.

29 CFR 1926.1051(a): Stairway(s)or ladder(s) were not provided at all personnel points of access where there was a break in elevation of 19 inches (48 cm) or more, or no ramp, runway, sloped embankment, or personnel hoist was provided: a) 15th floor, rebar holding rack: Employees climbed to the top of the rebar piles to tie rebar into bundles and sort the bundles on the rack. The height of the rack plus the rebar is approximately 3 feet 6 inches. Ladders were not provided to employees at access where there was a break in elevation of more than 19 inches. On or about 08/07/18.
