Guards were not affixed to the machine where possible and secured elsewhere if for any reason attachment to the machine is not possible: a) On or about August 21, 2018, Wolf Creek Dam grounds, employees were exposed to struck-by and amputation hazards while operating Exmark riding lawn mowers in that the employer did not ensure ejection chutes and belt/pulley guards were affixed. b) On or about August 21, 2018, Wolf Creek Dam grounds, employees were exposed to struck-by and amputation hazards while operating Stihl lawn trimmers (FS 240R) in that the employer did not ensure deflectors were affixed.
