29 CFR 1926.1053(b)(1): Portable ladders were used for access to an upper landing surface and the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface to which the ladder was used to gain access: Roofing and Gutter Solutions, LLC located at 5387 Long Drive, Timnath, CO 80547: On and preceding August 3rd 2018, the employer did not ensure that ladders used for access to an upper landing surface extended at least 3 feet above the upper landing surface to which the ladder was used to gain access. Employees were using an extension ladder to access the roof with approximately 6 inches extending above the landing surface. This exposed employees to falls from height.

29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b): Roofing and Gutter Solutions, LLC located at 5387 Long Drive, Timnath, CO 80547: On and preceding August 3rd 2018, the employer did not ensure that employees engaged in residential roofing activities 6 feet or more above lower levels were protected by guardrail systems, safety net systems, or personal fall arrest systems. Three employees were installing shingles on the roof of a two story residential home without the use of fall protection. The exposed employees to falls from height. Roofing and Gutter Solutions, LLC was previously cited for a violation of this occupational safety and health standard or its equivalent 29 CFR 1926.501(b)(13) which was contained in OSHA inspection number 1214873, citation number 1, Item 2, issued March 15th, 2017 and became a final order on April 10th, 2017, with respect to a workplace located at 4948 Wing Walker Drive, Colorado Springs, CO 80911.
