29 CFR 1910.95(c)(1):  A continuing, effective hearing conservation program as described in 29 CFR 1910.95(c) through (n) was not instituted when employee noise exposures equaled or exceeded an 8-hour time-weighted average sound level (TWA) of 85 dBA:       In the tank manufacturing facility, employees were exposed to continuous noise level at 91.6% of the allowable 8-hour time-weighted average sound level (85 dBA). The equivalent dBA level of 91.6% is approximately 89.4 dBA. The sampling was performed for 478 minutes during one shift on August 15, 2018. Zero exposure was assumed for the 2 minutes not sampled.

29 CFR 1910.1200(h)(1):  Employer did not provide employees with effective information and training on hazardous chemicals in their work area at the time of their initial assignment, and whenever a new chemical hazard the employees have not previously been trained about is introduced into their work area. Information and training may be designed to cover categories of hazards (e.g. flammability, carcinogenicity) or specific chemicals.
