29 CFR 1926.102(a)(1): The employer shall ensure that each affected employee uses appropriate eye or face protection when exposed to eye or face hazards from flying particles, molten metal, liquid chemicals, acids or caustic liquids, chemical gases or vapors, or potentially injurious light radiation.     On or about August 7, 2018, during the installation of roofing material at the job site located at 1832 Lincoln Way E., Massillon, Ohio 44646; the employer did not ensure employees working on the roof of a single family home were protected from possible danger of eye injury from impact, or from flying objects from the employees installing roofing material.       The TRI-STAR HOME IMPROVEMENTS, LTD was previously cited for a violation of this occupational safety and health standard or its equivalent standard (1926.102(a)(1)  Eye and face Protection), which was contained in OSHA inspection number 490999, citation number 01, item number 01 and was affirmed as a final order on 8/14/2014, with respect to a workplace located at 900 Medina Rd, Medina, Ohio 44215.

29 CFR 1926.20(b)(2): The employer did not initiate and maintain a safety program which provides for frequent and regular inspections of jobsites, materials, and equipment to be made by a competent person (i.e., a person who is capable of identifying exis:     On or about August 7, 2018, during the installation of roofing material on a single family home located at 1832 Lincoln way E., Massillon, Ohio 44646, the employer did not conduct frequent and regular inspections of the job site where that had 5 employees exposed to fall hazards, and flying debris from the use of air driven nail guns from working surfaces at heights approximately 9.4 feet above the ground.
