29 CFR 1910.119(j)(4)(ii): Inspection and testing procedures did not follow recognized and generally accepted good engineering practices : a. Penthouse 2A - On or about October 9, 2018, inspection procedures for a three way valve servicing an oil pot for the ammonia evaporators and 2 emergency relief valves did not follow their chosen recognized and generally accepted good engineering practices of IIAR in that excessive frost and icing build-up surrounded the exterior of the oil pot to include the handle of the three way valve had not been identified and corrected. Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date and method of abatement, and states that employees and their representatives were informed of this abatement.
