29 CFR 1926.451(c)(1): Supported scaffolds with a height to base width (including outrigger supports, if used) ratio of more than four to one (4:1) shall be restrained from tipping by guying, tying, bracing, or equivalent means: Located along the exterior area of the structure employees were observed working from a self-supported tubular welded frame scaffolding system with a 3 foot wide minimum base erected to a height in excess of 19 feet without the scaffolding system being secured to the structure or equivalent.

29 CFR 1926.452(c)(2): Frames and panels shall be braced by cross, horizontal, or diagonal braces, or combination thereof, which secure vertical members together laterally. The cross braces shall be of such length as will automatically square and align vertical members so that the erected scaffold is always plumb, level, and square. All brace connections shall be secured. Located along the exterior area of the structure employees were observed working from a self-supported tubular welded frame scaffolding system which was not completely protected by cross bracing or equivalent in that none of the interior sides of the frames adjacent to the structure had cross bracing installed.

29 CFR 1926.451(f)(7): Scaffolds shall be erected, moved, dismantled, or altered only under the supervision and direction of a competent person qualified in scaffold erection, moving, dismantling or alteration. Such activities shall be performed only by experienced and trained employees selected for such work by the competent person. Located along the exterior area of the structure the self-supported tubular welded frame scaffolding system was not erected or altered under the supervision of a competent person qualified in scaffold erection, in that cross bracing was not completely utilized, the 4 to 1 base to height was exceeded without securing the scaffold to the structure or equivalent and standard guardrails or personal fall arrest systems were not completely erected or utilized.
