29 CFR 1910.268(d)(1): Before work was begun in the vicinity of vehicular or pedestrian traffic which could endanger employees, warning signs, flags, barriers, and/or other traffic control devices were not placed conspicuously to alert and channel appropriate traffic: JOBSITE: The employer did not ensure through visual observation, specific instructions and other means that the work zone where an extension ladder was placed against a telephone pole in very close proximity from the roadway was appropriately protected from vehicular or pedestrian traffic by placing warning signs, flags, barriers, and/or other traffic control devices to alert and channel appropriate traffic while an employee was manually placing a "white flag" to mark a communication line from the extension ladder.
