29 CFR 1910.1200(e)(1): The employer did not develop, implement, and/or maintain at the workplace a written hazard communication program which describes how the criteria specified in 29 CFR 1910.1200(f), (g), and (h) will be met: (Construction Reference: 1926.59)  At a residential construction site located at 8809 NE 80th St in Kansas City, MO: Employees were exposed to harmful chemicals that could cause injuries and/or illnesses in that the employer had not developed, or implemented, a written hazard communication program. This was demonstrated at the residential construction site when 4 employees were dispensing gasoline that was used to power the GRIP RITE GR55G10X (Serial #11030662) air compressor for the pneumatic nail guns.

29 CFR 1926.25(a): During the course of construction, alteration, or repairs, debris was not kept cleared from work areas, passageways, and stairs, in and around buildings or other structures:  At a residential contruction site located at 8809 NE 80th St in Kansas City, MO: Employees were exposed to slip, trip and fall hazards. This was demonstrated when the employer allowed construction materials and debris to accumulate around the building and in areas being used by employees to access elevated areas of the building. Debris also contained protruding nails that could injure employees if they were to step on them or fall.

29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed:   A worksite located at 8809 NE 80th St, Kansas City, MO: Employees were exposed to a fall hazards. Employees were using Louisville and Werner brand extension ladder to access the roof of the residential worksite. The extensions ladders were not extended at least 3 feet above the upper landing surface. One ladder was place at the eve of the roof and two others were only placed one foot above the upper landing of the roof.
