29 CFR 1926.501(b)(1): Each employee on a walking/working surface with an unprotected side or edge which was 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems.    a)  At the jobsite, on the 4th floor, on or about 8/7/18:  Workers were chipping away concrete where the employees were working in close proximity to an unprotected gap between the walking-working surface and the pre-existing concrete floor, exposing them to falls of 10 feet.     b)  At the jobsite, on the ramp between the 2nd and 3rd floors, cable guard-rails were missing from a wall opening where the knee-wall was 2 feet high, exposing workers to falls of 12 feet.      c)  At the jobsite, on the 3rd floor, cable guard-rails were dropped and laying on the floor where the knee-wall was 2 feet high, exposing workers to falls of 30 feet.
