29 CFR 1910.134(c)(1): A written respiratory protection program that included the provisions in 29 CFR 1910.134(c)(1)(i) - (ix) with worksite specific procedures was not established and implemented for required respirator use:   At this precast concrete shop, the employer had not established or implemented a respiratory protection program when employees were required to wear tight-fitting respirators when using the hazardous chemicals and materials for mixing, spraying, painting, sanding, grinding structural metal and pouring concrete products.  An effective respiratory protection program would include, but not limited to:  a. Procedures for selecting respirators for use in the work place; b. Medical evaluation of employees required to wear respirators; c. Valid fit testing procedures for each type of tight-fitting respirators; d. Procedures and schedules for cleaning, disinfecting, storing, inspecting, repairing, discarding and maintaining respirators, and; e. Training of employees in the proper use of respirators.

The employer did not perform frequent inspections for deficiencies for the following overhead cranes:  a. Overhead crane (Kranco Houston, 5 Ton) such as but not limited to rusted cables/components; and, b. Overhead crane (Proserv 10 Ton) such as but not limited to rusted cables/components.

29 CFR 1910.253(b)(4)(iii): Oxygen cylinders in storage were not separated from fuel-gas cylinders or combustible materials (especially oil or grease), a minimum distance of 20 feet (6.1 m) or by a noncombustible barrier at least 5 feet (1.5 m) high having a fire-resistance rating of at least:   On or about January 31, 2018, at this plastic mold injection plant employees used compressed oxygen and fuel gas cylinders for welding and torching, and the compressed gas cylinders were not separated when not in use.   The Victoria Precision Products, Inc. was previously cited for a violation of this Occupational Safety and Health standard, which was contained in OSHA inspection number 1241729, Citation 1, Item 1, and was issued on October 4, 2017, and was affirmed as a final order on October 19, 2017, with respect to a workplace located at 1511 Garcitas Creek Road, Inez, TX 77968.

29 CFR 1910.1200(e)(1): The employer did not develop, implement, and/or maintain at the workplace a written hazard communication program which describes how the criteria specified in 29 CFR 1910.1200(f), (g), and (h) will be met:  At this precast concrete shop, employees were using the hazardous chemicals and materials for mixing, spraying, painting, sanding, grinding structural metal and pouring concrete products. The employer did not developed a program which would address labeling and other forms of warning on chemicals containers, Safety Data Sheets; employees information, and training on the hazards associated with chemicals used at this site.  The written program must also contain the following:  a. a list of all hazardous chemicals on site; b. the methods the employer will use to inform employees of the hazards associated with non-routine tasks involving       chemicals, such as a spill; c. the hazards of chemicals contained in piping that is not labeled, and; d. the method the employer will use to inform other employers (contractors) of the chemicals their employees might        be exposed to while performing duties at this site.
