On or about August 9, 2018, at the above addressed jobsite, employees using a pneumatic nail gun did not wear safety glasses, thereby exposing the employees to the hazards associated with eye injuries.

On or about August 9, 2018, at the above addressed jobsite, employees performing residential roofing operations on a roof ten (10) feet two (2) inches above the ground were not protected from falls, thereby exposing employees to fall hazards.

On or about August 9, 2018, at the above addressed jobsite, employees had been provided fall protection training, however, the employer failed to keep a written certification of record.
