29 CFR 1926.451(e)(1): When scaffold platforms were more than 2 feet (0.6m) above or below a point of access the employer did not provide one of the following; portable ladders, hookon ladders, attachable ladders, stair towers (scaffold stairways/towers), stairwaytype ladders (such as ladder stands), ramps, walkways, integral prefabricated scaffold access, or direct access from another scaffold. a) Safe access to scaffold was not provided; employer was having employees climb down 8 feet from the roof to get on the suspended scaffold (manual rope-pulley scaffold) while the employer was holding the pulley ropes. b) When going onto the suspended scaffold from the roof the employee did not tie off to the lifeline until standing on the roof edge exposed to 50 ft fall hazard. Location: 1330 5th Ave, New York NY On or about 7/30/2018 NOTE: BECAUSE ABATEMENT OF THIS VIOLATION IS ALREADY DOCUMENTED IN THE CASEFILE, THE EMPLOYER NEED NOT SUBMIT CERTIFICATION NOR DOCUMENTATION OF ABATEMENT OF THIS VIOLATION AS NORMALLY REQUIRED BY 29 CFR 1903.19.
