On 8/8/2018, at the Lithonia facility, the employer did not ensure process safety information was available for the following equipment: a - C-05 Oil Separator b - C-06 Oil Separator c - V-03 Intercooler Oil Pot d - Engine Room fan the fan in use at the facility was NOT non-sparking materials and instead was 18 ga. galvanized steel.

29 CFR 1910.120(e)(3)(iii): Workers regularly on site who worked in areas which had been monitored and fully characterized indicating exposures were under permissible exposure limits and/or published exposure limits where respirators were not necessary, and the characterization indicated that there were no health hazards or the possibility of an emergency developing, did not receive a minimum of 24 hours of instruction off the site, and the minimum of one day actual field experience under the direct supervision of a trained, experienced supervisor On 8/8/18, at the Lithonia facility, the employer did not ensure employees who were expected to respond to releases received refresher training in a timely fashion when they were expected to respond to ammonia releases at the facility. Employees originally trained at the facility last had 8-hr. refresher training 2/8/17.
