The employer had not trained any employees in the safe operation of operating a powered industrial truck.

At the Commercial construction site located at Building #2 Forest Ave Liberty, MO 64068: An employee were exposed to fall hazards when any form of fall protection was not provided to employees working above 6 feet on a commercial construction site. An employee was unloading drywall from a forklift on a third story of a three floor apartment building by way of the balconies; these balconies did not have a guardrail in place to protect the employees from falling approximately 30 feet to the ground.
