29 CFR 1904.29(a): The employer did not use an OSHA 300, 301 and 300A Form or equivalent. a) 600 Church St., Norfolk, VA 23501, PDC: On or about August 16, 2018, and times prior thereto, the employer's workers compensation forms used for recording injuries and illnesses were not equivalent forms for calendar years 2013, 2014, 2015, 2016, 2017 and 2018. Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date and method of abatement, and states employees and their representatives were informed of this abatement.

29 CFR 1904.41(a)(2): Annual electronic submission of OSHA Form 300A Summary of Work-Related Injuries and Illnesses by establishments with 20 or more employees but fewer than 250 employees in designated industries. The establishment had 20 or more employees but fewer than 250 employees at some time during the previous calendar year, and the establishment was classified in an industry listed in appendix A to subpart E of this part, but did not electronically submit information from OSHA Form 300A Summary of Work-Related Injuries and Illnesses to OSHA or OSHA's designee. The employer did not submit the information once a year, no later than the date listed in paragraph (c) of this section of the year after the calendar year covered by the form: a) 600 Church St., Norfolk, VA 23501, PDC: On or about August 16, 2018, and times prior thereto, the employer failed to electronically submit information from the OSHA Form 300A or equivalent by July 1, 2018. 29 CFR 1910.305(g)(1)(iv)(A): Flexible cords and/or cables were used as a substitute for the fixed wiring of a structure: 600 Church St., Norfolk, VA 23501, PDC: On or about August 18, 2018, the employer used a flexible (SO) cord drop from a ceiling-mounted electrical box: a) to power a motor control panel on a low-cost tray sorting machine. b) to power an automatic tray unsleaver, exposing employees to potential shock and fire hazards. Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date and method of abatement, and states employees and their representatives were informed of this abatement.
