29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):    a) Northwest panel of roof, on or about 8-10-18:  Five employees were working on a 6:12 pitch roof and were not protected from falls, exposing the employees to a fall hazard of 21 feet.   ABATEMENT CERTIFICATION IS REQUIRED FOR THIS ITEM

29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed:     a) West and north sides of building, on or about 8-10-18:  Five employees were accessing various panels of roof via one of four portable extension ladders that were not extended 3 feet above the landing surface, exposing the employees to fall hazards of 21 feet, 13 feet, and 8 feet.
