29 CFR 1926.454(a): The employer did not have each employee who performed work while on a scaffold trained by a person qualified in the subject matter to recognize the hazards associated with the type of scaffold being used and to understand the procedures to control or minimize those hazards.  (a) The employer did not ensure that employees received training in recognizing fall hazards and implementing fall protection when conducting work from an elevated platform. At this worksite, there was a wooden platform that was used for the framing carpentry work.

The platform was resting on the forks of a telehandler forklift and was used to support persons, material and equipment during the framing carpentry process on a residential building.
