29 CFR 1910.305(g)(2)(iii): Flexible cords were not connected to devices and fittings so that tension would not be transmitted to joints or terminal screws: At the establishment, employees used a black extension cord that had the outer protective sheating pulled away and was not connected with a strain relief device to prevent tension exposing employees to the hazard of electrical shock and fire hazards.
