Jobsite: The employees overlaying a new EPDM synthetic rubber roofing membrane over the existing roofing material were not protected from falling approximately 30 feet to the atrium floor through un-protected skylights.

Or, on roofs 50-feet (15.25 m) or less in width, each employee was not protected by use of a monitoring system. Jobsite: The employer did not ensure that each employee re-roofing a flat roof was protected from fall hazards approximately 30 feet to the asphalt paved ground by the use of appropriate fall protection systems/devices.
