29 CFR 1910.212(a)(3)(ii): Point of operation guards were not designed and constructed as to prevent the operator from having any part of their body in the danger zone during the operating cycle. In the forming machine, the employer did not ensure a guard was installed on the bottle making machine to protect employees from point of operation of operation hazards exposing employees to caught-in and in-going nip-points hazards.

29 CFR 1910.212(a)(3)(iii): Special hand tools for placing and removing material were not provided to permit easy handling of material without the operator placing a hand in the danger zone. In the forming machine area, employees were not using appropriate tools for adjusting the parameters of the coolant on machine A3 and were exposed to amputation hazards.
