29 CFR 1910.147(c)(4)(ii): Energy control procedures did not clearly and specifically outline the scope, purpose, authorization, rules, and techniques to be utilized for the control of hazardous energy, and the means to enforce compliance including, but not limited to, 29 CFR 1910.147(c)(4)(ii)(A), (c)(4)(ii)(B), (c)(4)(ii)(C) and (c)(4)(ii)(D): On or about 08/13/2018, Empire Fence & Custom Iron Works, located at 4001 Taft Ave., St. Louis, MO, failed have developed, documented and utilized proper/adequate procedures for the control of potentially hazardous energy for following machines located in the plant: Hypertherm Plasma Cutting Table (230 V electrics, 110 V electrics, and 100 psi pneumatics (air); Eisele saw (230 V electrics and hydraulics).
