29 CFR 1926.102(a)(1): Eye and face protective equipment were not used when machines or operations presented potential eye or face injury from physical, chemical, or radiation agents:  On or about 08/09/2018, at the jobsite at 1395 Covington Manor Lane, St Louis, MO, employees were applying shingles using a Hitachi Nail Gun, Serial #NV45AB2, 1 3/4 Coil Nailer,  Air Pressure 70 - 120 psi, and were not wearing safety glasses.

29 CFR 1926.501(b)(1):   Each employee on a walking/working surface having an unprotected side or edge which was six or more feet (1.8m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems:   On or about 08/09/2018, at the jobsite at 1395 Covington Manor Lane, St Louis, MO, employees engaged in roof repair/installation operations were not provided conventional fall protection.  Harnesses were at the jobsite; however, they were not worn by roofers.  Eave to ground was approximately 9 feet 3 inches.

29 CFR 1926.1053(b)(1): Where a portable ladder was used for access to an upper level landing surface and the ladder's length did not allow the ladder side rails to extend at least 3 feet (0.9 m) above the upper landing surface being accessed, the ladder was not secured at its top to a rigid support that will not deflect, and/or a grasping device, such as a grab rail, was not provided to assist employees in mounting and dismounting the ladder:  On or about 08/09/2018,  at the jobsite at 1395 Covington Manor Lane, St Louis, MO, workers were using an aluminum ladder to gain access to the roof, the ladder's length did not allow the ladder side rails to extend at least 3 feet above upper landing surface, the ladder was not secured at its top to a rigid support that will not deflect, and/or a grasping device, such as a grab rail, was not provided to assist employees in mounting and dismounting the ladder.  The workers were exposed to a fall hazard.
