29 CFR 1917.111(d): All walking and working surfaces in the terminal areas were not maintained in good repair: (a) on or about 7/31/2018, and at times prior thereto, employees working on the roof of the middle grain silo at the Umatilla facility were exposed to tripping hazards in that the expanded metal bridge had protrusions which could ensnare boots or shoes.
