OSH ACT of 1970 Section (5)(a)(1): The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to falling loads:  a) Main Bay South, 858 New Castle Road, Butler, PA 16001:  On or about September August 13, 2018, a periodic inspection of Crane #1 (bridge manufacturer:  Keystone Crane and Hoist, serial number 912056, 5 ton capacity) had not been conducted within the past twelve months.  The last periodic inspection of Crane #1 occurred on December 9, 2016.  b) Main Bay Center, 858 New Castle Road, Butler, PA 16001:  On or about September August 13, 2018, a periodic inspection of Crane #2 (bridge manufacturer:  Keystone Crane and Hoist, 5 ton capacity) had not been conducted within the past twelve months.  The last periodic inspection of Crane #2 occurred on December 9, 2016.  c) Main Bay North, 858 New Castle Road, Butler, PA 16001:  On or about September August 13, 2018, a periodic inspection of Crane #3 (bridge manufacturer: Dresser Industries, serial number 912056, 5 ton capacity) had not been conducted within the past twelve months.  The last periodic inspection of Crane #3 occurred on December 9, 2016.

29 CFR 1910.147(c)(5)(i): Locks, tags, chains, wedges, key blocks, adapter pins, self-locking fasteners, or other hardware were not provided by the employer for isolating, securing, or blocking of machines or equipment from energy sources:      a) Production area, 858 New Castle Road, Butler, PA 16001:  On or about August 8, 2018, locks or other hardware required for locking out an electrical panel breaker switch were not provided by the employer.  The procedure for locking out the Acromix Systems batching/mixing system (model number:  BR-20210, serial number SN-74565ER021605080) required the main breaker to be locked out.

Employees required to clean cement from the batching/mixing system were not trained as Authorized employees under the employer's energy control program.
