29 CFR 1926.754(e)(5)(i): Except as provided in � 1926.760(c), metal decking shall be laid tightly and immediately secured upon placement to prevent accidental movement or displacement. On August 9th 2018 and prior to that time located at the narthex area low entry roof at or adjacent to column line 7.5 & F.1 employees who were not working in an established control decking zone (CDZ) and who were involved in steel erection activities were placing and screwing clips through insulation into purlins on the structural steel structure to install metal roof sheeting and installing metal roof sheeting while walking on the metal roof sheeting which was not secured upon placement to prevent accidental movement or displacement exposing employees to fall potentials in excess of 20 feet.

On August 9th 2018 and prior to that time located at the narthex area low entry roof at or adjacent to column line 7.5 & F.1 employees who were not working in an established control decking zone (CDZ) and who were involved in steel erection activities were placing and screwing clips through insulation into purlins on the structural steel structure to install metal roof sheeting and installing metal roof sheeting while walking the metal roof sheeting which during initial placement did not have the metal roof sheeting decking panels placed to ensure full support by structural members exposing employees to fall potentials in excess of 20 feet.

29 CFR 1926.760(d)(1): Guardrail systems, safety net systems, personal fall arrest systems, positioning device systems and their components shall conform to the criteria in � 1926.502. On August 9th 2018 and prior to that time located at the narthex area low entry roof at or adjacent to column line 7.5 & F.1 employees who were not working in an established control decking zone (CDZ) and who were involved in steel erection activities were placing and screwing clips through insulation into purlins on the structural steel structure to install metal roof sheeting and installing metal roof sheeting while walking on the metal roof sheeting who were utilizing personal fall arrest systems and their components that did not conform to the criteria in 1926.502(d)(16)(iii) in that when stopping a fall, the personal fall arrest systems and their components were not rigged such that an employee can neither free fall more than 6 feet, nor contact any lower level exposing employees to fall potentials in excess of 20 feet and striking the ground below.
