29 CFR 1910.25(b)(3): The employer shall ensure:  Stairs have uniform riser heights and tread depths between landings;      (a)  The two metal grated steps leading up to both sides of the North and South Annex had risers that were not uniform in height in that the risers were 6 1/2 inch, 7 inch and 8 inch,  and 6 1/2 inch, 7 inch and 10 inch, creating a fall hazard for employees using the steps to enter the Annex.

29 CFR 1910.36(g)(2): An exit access must be at least 28 inches (71.1 cm) wide at all points. Where there is only one exit access leading to an exit or exit discharge, the width of the exit and exit discharge must be at least equal to the width of the exit access.    a.  On August 14, 2018, in the North Annex Pre-Load Area, unloaders in the Annex were exposed to a fire, entrapment, and emergency evacuation hazards by a roller extension unloader device that was permanently located and attached to the belt conveyor and that limited the exit access route to 20 inches.      b.  On August 14, 2018, in the North and South Annex Pre-Load Area when Portable Distribution Centers (PDCs) were attached, unloaders were exposed to a fire, entrapment and emergency evacuation hazards by a crank handle that extends the rollers on the belt conveyor into the PDC and limited the exit access route to 7 inches.        The United Parcel Service, Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard [29 CFR 1910.36(g)(2)], which was contained in OSHA inspection number 956866, citation number 1, item number 1 and was affirmed as a final order on May 12, 2016, with respect to a workplace located at 8200 Excellence Place, Belleville, IL 62223.    The United Parcel Service, Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard [29 CFR 1910.36(g)(2)], which was contained in OSHA inspection number 1014793, citation number 1, item number 1a and was affirmed as a final order on April 8, 2015, with respect to a workplace located at 2014 East Olive Street, Decatur, IL 62525.

29 CFR 1910.37(a)(3): Exit routes must be free and unobstructed. No materials or equipment may be placed, either permanently or temporarily, within the exit route. The exit access must not go through a room that can be locked, such as a bathroom, to reach an exit or exit discharge, nor may it lead into a dead-end corridor. Stairs or a ramp must be provided where the exit route is not substantially level.    (a)  On June 26, 2018, in the entrance to the North Annex Pre-Load Area, unloaders were exposed to a fire, entrapment and emergency evacuation hazards due to the employer not ensuring that the metals steps leading in and out of the North Annex were unobstructed from irregular packages and other sized packages stacked in front of the steps.      (b)  On July 3, 2018, in the North Annex Pre-Load Area, unloaders were exposed to a fire, entrapment, and emergency evacuation hazards in that the employer did not ensure that the aisle ways leading to the emergency exits were unobstructed due to the accumulation of irregular and other sized packages in the aisle way.     (c)  On July 20, 2018, in the North Annex Pre-Load Area, unloaders and drivers were exposed to a fire, entrapment, and emergency evacuation hazards in that the employer did not ensure that the aisle ways leading to the emergency exits were unobstructed due to the accumulation of irregular and other sized packages in the aisle way.      (d)  On August 14, 2018, In the North Annex Pre-Load Area, unloaders and drivers were exposed to a fire, entrapment, and emergency evacuation hazards in that the employer did not ensure that the aisle ways leading to the emergency exits were unobstructed due to the accumulation of irregular and other sized packages in the aisle way.      (e)  On August 28, 2018, in the North Annex Pre-Load Area, unloaders were exposed to a fire, entrapment, and emergency evacuation hazards in that the employer did not ensure that the aisle ways leading to the emergency exits were unobstructed due to the accumulation of irregular and other sized packages in the aisle way.      (f)  On August 30, 2018, in the North Annex Pre-Load Area, unloaders were exposed to a fire, entrapment, and emergency evacuation hazards in that the employer did not ensure that the aisle ways leading to the emergency exits were unobstructed due to the accumulation of irregular and other sized packages in the aisle way.      (g)  On August 31, 2018, in the North Annex Pre-Load Area, unloaders were exposed to a fire, entrapment, and emergency evacuation hazards in that the employer did not ensure that the aisle ways leading to the emergency exits were unobstructed due to the accumulation of irregular and other sized packages in the aisle way.      United Parcel Service, Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard [1910.37(a)(3)], which was contained in OSHA inspection number 1011552, citation number 1, item number1, and was affirmed as a final order on July 8, 2016, with respect to a workplace located at 3 Prosperity Drive, Savannah, GA 31408.    United Parcel Service, Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard [1910.37(a)(3)], which was contained in OSHA inspection number 896766, citation 2, item number 1 and was affirmed as a final order on February 27, 2014, with respect to a workplace located at 493 County Avenue, Secaucus, NJ 07094.

29 CFR 1910.37(b)(5): Each doorway or passage along an exit access that could be mistaken for an exit must be marked "Not an Exit" or similar designation, or be identified by a sign indicating its actual use (e.g., closet).    (a)  On August 14, 2018, in the Portable Distribution Center (PDC)  that was attached to the North and South Annex had two doors on the end of the PDC that could be mistaken for an exit and were not marked as "Not an Exit".
