29 CFR 1910.178(l)(1)(i): The employer did not ensure that each powered industrial truck operator is competent to operate a powered industrial truck safely, as demonstrated by the successful completion of the training and evaluation specified in this paragraph (l): On or about August 7, 2018 at the Camping World Facility located at 3001 E. Jefferson Road in Grain Valley, Missouri- Employees were exposed to serious caught-in, crushing and or struck-by injuries, in that the forklift operators had not completed a competent training program.

29 CFR 1910.305(d)(2): Panelboard enclosures. Panelboards shall be mounted in cabinets, cutout boxes, or enclosures designed for the purpose and shall be dead front. However, panelboards other than the dead front externally-operable type are permitted where accessible only to qualified persons. On or about August 7, 2018 at the Camping World facility located at 3001 E. Jefferson Road in Grain Valley, Missouri- Employees in the job shop were exposed to hazard of electrocution from open areas within an electrical panel which did not provide a protection from inadvertent contact with exposed live electrical connections.
