On or about August 13, 2018, at the above referenced inspection site, the employer failed to ensure that all work areas and walking-working surfaces were kept free of accumulated dust. Employees were thereby exposed to slip/trip/fall and fire hazards.   c. On or about August 13, 2018, at the above referenced inspection site, the employer failed to ensure ladders were orderly stored on the second floor mezzanine where maintenance employees worked. Employees were thereby exposed to head injuries from the low clearance created by a ladder left leaning across a walkway.   There is no abatement certification of documentation required for this item.

A metallic single gang box listed for wall mounting was used, with missing knockouts, at the end of a job made extension cord / pendant drop. Employees handling the box were thereby exposed to fire and shock hazards.   There is no abatement certification of documentation required for this item.
