29 CFR 1926.501(b)(10): Each employee engaged in roofing activities on low-slope roofs with unprotected sides and edges 6 feet or more above lower levels, was not protected from falling by guardrail systems, safety net systems, personal fall arrest systems, or a combination of warning line system and guardrail system, warning line system and safety net system, or warning line system and personal fall arrest system, or warning line system and safety monitoring system: (a) SOUTHEAST CORNER, SOUTH SIDE - Employee(s) installed roofing material on the entryway roof, and the employee9s) was not protected by any type of fall protection, which thereby exposed the employee(s) to a 12 foot fall hazard, on or about August 9, 2018. (b) NORTHWEST CORNER, WEST SIDE - Employee(s) were cleaning up roofing materials, and the employee(s) were exposed to a 14 foot section where the warning line system had been removed, which thereby exposed the employee(s) to a 13 foot fall hazard, on or about August 9, 2018. Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date and method of abatement, and states employees and their representatives were informed of this abatement.

29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed: (a) SOUTHEAST CORNER OF STRUCTURE, SOUTH WALL - Employee(s) accessed the entryway roof by using a 32 foot extension ladder, and the rails of the ladder were only positioned approximately 30 inches past the upper landing surface, on or about August 9, 2018. No abatement certification or documentation required.
