29 CFR 1926.501(b)(11): Each employee on a steep roof with unprotected sides and edges 6 feet (1.8 m) or more above lower levels was not protected from falling by guardrail systems with toeboards, safety net systems, or personal fall arrest systems:  Site- Employees working from the roof were not protected from a fall of approximately 22 feet where fall protection was not utilized. On or about August 11, 2018 an employee fell from the roof and sustained severe injuries where fall protection was not utilized.

29 CFR 1926.503(b)(1): The employer did not verify compliance with paragraph (a) of this section by preparing a written (training) certification record including the name or other identity of the employee trained, the date(s) of the training, and the signature of the person who conducted the training or the signature of the employer.  Site- The employer did not have written documentation of fall protection training for each employee exposed to a fall hazard. On or about September 21, 2018 the employer's representative stated in writing LSG has no such documents in its possession, custody, or control.

29 CFR 1904.39(a)(2): The employer did not report within 24-hours a work-related incident resulting in in-patient hospitalization, amputation or the loss of an eye.  Site- On or about August 11, 2018 an employee sustained a serious fall injury which required an in-patient hospitalization of the employee and the in-patient hospitalization was not reported by the employer to OSHA within 24 hours.
