On or about August 14, 2018 at jobsite located at 1240 S Pine Island Rd, Plantation FL 33324, employees were exposed to engulfment hazards due to spoil pile located less than 2 feet from the edge of the trench.

On or about August 14, 2018 at jobsite located at 1240 S Pine Island Rd, Plantation FL 33324, employees were exposed to engulfment hazards due to working inside a 5.93 foot deep trench without cave in protection.
