29 CFR 1910.28(b)(1)(i)(A): The employer did not ensure that each employee on a walking-working surface with an unprotected side or edge that is 4 feet (1.2 m) or more above a lower level was protected from falling by a standard guardrail system.    Office Mezzanine: A walkway approximately 8 feet from ground level did not have a standard guardrail, with a top rail between 39 and 45 inches and a mid-rail midway between top rail and walking surface. The top rail along the open side measured 32 inches and there was no mid-rail installed.

On 8-3-18 a worker suffered burns when a hose spilt, splattering the heated liquid onto his forearms.
