Installed Building Products of Toledo dba Royalty Mooney and Moses located in Northwood, Ohio: On or about August 15, 2018, the employer did not ensure employees required to wear tight fitting half face respirators had received a medical evaluation prior to wearing the respirators to perform the spray foam application.
